All referrals should be made through the Executive Director at the Main Office.
Referrals can be made in writing or by phone. Referrals can be made by anyone this includes self-referrals. After a referral is made the Executive Director will interview the survivor, his or her family, and any involved therapists, social workers, funding agents, lawyers etc. This initial interview will determine the suitability of the individual to the programs as well as assist in determining levels of support required. Follow up pre-admission interviews may also be necessary by other members of the Cheshire Team.

Prior to admission a review of current medical records will take place and a decision made as to the most appropriate residential facility placement and a possible date of intake. Candidates and their families will then be encouraged to tour the facility prior to committing to the program. Funding sources must also be identified at this time.

Note: The intake process can be lengthy and is dependent on bed availability as well as funding clarification. Questions with respect to wait lists can be made to the Executive Director.

An Intake meeting with the resident, friends/family, and involved professionals will take place to review house rules and expectations, sign consent forms, gain emergency information and answer any questions. Once moved in, the resident will be expected to take part in a life skills assessment (typically 6 - 8 weeks in duration). During the assessment period an initial plan will be developed that includes recreational pursuits, an exercise regime, medical appointments, and referrals to other professional services.

Once the assessment is complete a team meeting will take place with the resident, family/friends, the funding source and involved professionals. The individual plan and assessment results will be reviewed, future goals identified as well as recommendations for further therapy. Future meeting dates will be set which generally occur every three to four months.
