The Cheshire Homes Society provides in house rehabilitation support staff to clients living in their own homes. Support options can be customized to meet an individual’s specific needs through the development of an individual service plan. Once a program has been established, team meetings involving all interested parties take place on a regular basis to review the goals and service plan. Where an individual gains independence in their daily living skills new goals are established and support levels are adjusted accordingly. The Society has provided this type of ongoing support to a number of individuals over the past twenty years.
